Release
I acknowledge that I am willingly participating in these activities and that I have assumed all risks as described above. In consideration for my being allowed to participate in the activities offered, I, the undersigned hereby release Pure Synergy, LLC, their owner, instructors and employees from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with my participation in this activity, including those allegedly attributed to the negligent acts or omissions of the above mentioned parties. This agreement shall be binding upon me, my successors, heirs, executors, or transferees.
If any portion of this agreement is held invalid, I agree that the remainder of the agreement shall remain in full legal force and effect. If I am signing on behalf of a minor child, I also give full permission for any person connected with Pure Synergy, LLC to administer first aid deemed necessary, and in case of serious illness or injury, I give permission to call for medical care for the child and to transport the child to a medical facility deemed necessary for the well-being of the child.
Cancellation Policy - Massage/Bodywork
Any massage/bodywork/energywork appointment cancellations with less than 24 hours of notice are subject to a cancellation fee of 50% of the appointment cost. Clients who miss their appointments without giving any prior notification will be charged in full for the scheduled service. We recognize the time of our clients and staff is valuable and have implemented this policy for this reason. When you miss an appointment with us, our therapists not only lose your business, but also the potential business of other clients who could have scheduled an appointment for the same time. Additionally, many times our staff will be functioning in an “on call” status and may have traveled to the office specifically for your service. For these reasons, we are obligated to compensate our staff for their time.
Cancellation Policy - Group Classes
Pre-registration is required for group classes and a class may be cancelled if there are no attendees registered for 2 hours prior to it's start time.
Any group class reservation cancellations with less than 4 hours of notice (or no-show) are subject to a cancellation fee of $10 or a class credit.
Arrive to class 5-10 minutes early to prepare for class. For the safety of our students and instructors, the front door will be locked at the start of class. Preparation includes: checking in, turning off your cell phone, using the restroom before class (to avoid having to leave during the class), talking to the instructor about any injuries you might be dealing with, quietly sitting on your mat preparing mentally for the class to start. Do not eat immediately before class, and be sure to hydrate well before coming to the studio!
Cancellation Policy - Workshops/Events
We will require payment upfront. This can be done from your account, or someone will be in contact with you to collect payment.
Registrations canceled 10 days or less before the date of the workshop will receive the amount paid as a credit on their account usable for services, classes, or products. Registrations canceled with less than 24 hours before the workshop will receive 50% of the amount paid as a credit on their account usable for services, classes, or products. Registrations that miss the workshop without giving prior notification will be charged in full for the workshop and receive no credit.
Cancellation Policy - Membership
For the discounted rate of $85 per month, we ask for payments setup on the 1st of the month and give us a 30 day notice in writing via email to hello@puresynergydfw.com to cancel or pause. If you get sick/hurt/travel, we can hold your membership for up to 2 months. If you cancel within 12 months of starting, we require a $50 fee to restart within that same 12 months. Basically we started that when we took away our 12 month contract, in lieu of a cancellation fee.
No refunds are allowed for past months due to the customer’s failure to utilize online services or failure to pause or cancel their membership.
Gift Certificate
All gift certificates/ gift cards are transferable but non-refundable.
Massage and bodywork agreement
It is my choice to receive massage therapy. I am aware of the benefits and risks of massage and give my consent for massage. I understand that there is no implied or stated guarantee of success of effectiveness of individual techniques or series of appointments. I acknowledge that massage therapy is not a substitute for medical care, medical examination or diagnosis. I have stated all medical conditions that I am aware of and will inform my practitioner of any changes in my health status.
The Texas Administrative code, Title 25, Part 1, Chapter 140, Subchapter H, Rule 140.304 states that this initial consultation document is required and that it must include the following information:
• Pure Synergy, LLC will not preform breast massage on female clients.
• Draping will be used during the session. “Draping” means that your body will be modestly covered by a sheet during the massage.
• For any reason, the client may ask the therapist to cease the massage, and the therapist will do so. The therapist also reserves the right to terminate the session in the event of any sort of abusive behavior from the client. If client misbehavior should result in an abbreviated session, the client will be expected to render full payment.
Photo agreement
I authorize Pure Synergy, LLC permission to use my photograph or video in any and all of its publications including but not limited to printed and digital publications. I understand that any photograph or video of me will become property of Pure Synergy, LLC and that I will not be compensated for the use of said photographs or video content. – For classes / workshops / events ONLY. Never for massage services.
